Helpful Information for Existing Patients

Rescheduling or cancelling your appointment:

  • Please call the office at 518-782-3815 for rescheduling or cancelling your appointment, reminder of our 24 hour cancellation policy.

Medication/Prescription Refills:

  • PLEASE ALLOW UP TO 72 HOURS FOR ALL MEDICATION REFILL REQUEST TO BE FILLED. After you complete the prescription refill request form, it will be sent to Updox via a secure link. You can opt to call 518-782-3815 and leave a message for a medication refill request. 

  • If your refill request was submitted, please consider reaching out to the pharmacy before calling the office to follow up on the status of your medication refill request. The medication may be waiting at the pharmacy. Most pharmacies have an option to notify you by text.

  • If the pharmacist tells you he/she will call me; please call me too.  This will prevent a miscommunication or a delay in you receiving your medication.

  • Controlled substance medication refill requests.  Please complete the medication online form or call the office within 72 hours of the medication refill due date.  Most pharmacies are not able to refill medications early due to NYS/FDA guidelines. The FDA will indicate a shortage controlled substances from time to time. 

  • Reminder; if you request a medication refill for a 90 days supply, your insurance may dispense a 30 day supply (please check insurance policy for this information).

Updox

It’s important that your personal information be kept confidential. For all patient communication, we use a tool called Updox. HIPAA compliant communication platform for me to provide secure communication with my patients which communicates with my EHR. This system includes fax and telehealth communication. We use this system for telehealth visits and for sending forms.
