NEW PATIENTS
What to Expect at Your Appointment
Welcome!
Here’s what you can expect when you make an appointment:
An in person meeting for the initial appointment is preferred.
The appointment will likely take 45-60 minutes.
Please be aware that the initial appointment is not a guarantee of medication management and medication may not be prescribed at the first appointment.
You will be required to fill out all online new patient forms and submit via updox (a secure message system) within 24 hours PRIOR to your appointment.
All new patient forms will be reviewed PRIOR to your first appointment.
Please verify your health insurance coverage PRIOR to your appointment. Please bring your insurance card to your appointment.
Payment (out of pocket) and/or copays will be collected at the time of the appointment.
To cancel or reschedule your appointment please call 518-782-3815.
On occasion my dog, Gabbi, who is an emotionally intuitive support dog, may be in the office at the time of your visit. Her job is to take care of YOU. The session is about YOU and NOT Gabbi. You may elect not to have her present during your visit.
Updox
It’s important that your personal information be kept confidential. For all patient communication, we use a tool called Updox. HIPAA compliant communication platform for me to provide secure communication with my patients which communicates with my EHR. This system includes fax and telehealth communication. We use this system for telehealth visits and for sending forms. If you receive a message on your cell phone/email “You have a message from your medical provider…” it’s from us!
