  • What is the fee for the service charge (private pay)?

  • Your first appointment will be in person and likely take 45-60 minutes, but may run longer. It’s an opportunity to assess your specific needs, to answer your questions, and in some instances, formulate a working diagnosis and to collaboratively develop a plan of care. Occasionally, more than one appointment is necessary to conduct a full assessment and formulate treatment recommendations. The more complete your intake questionnaire is, the more likely objectives are achieved at our first visit. Please bring your insurance card and state issued identification to the initial appointment.

  • Yes, we offer in person and remote/virtual appointments, however it is preferred that the initial appointment is in person.

  • You will receive a secure text/email to connect via UPDOX telehealth platform typically 15 minutes prior to your session.

  • Yes, we accept 3 insurances: CDPHP commercial insurance, Highmark (Blue Shield of Northeastern NY) and Medicare.

    It is the patient’s responsibility to know the specifics of their insurance plan, including the status of any deductible, copays or coinsurance. Please contact your insurance carrier directly for that information. Patients that have other insurance plans not listed above, may still be able to use their insurance if the plan has out-of-network benefits. However, it is the patient’s responsibility to inquire regarding their insurance’s handling of out-of-network benefits, and the patient will be responsible for payment in full at time of service. A superbill can be provided at patient’s request, which then can be sent to their insurance carrier for out of network benefits.

  • It is an itemized list of services for the patient. The patient can submit this to their health insurance plan for reimbursement for out-of-network benefits.

  • Current credit card information is required to be kept on file for all patients. This information will be used to pay for the full cost of unreimbursed services and for fees, including those related to refill requests made outside of scheduled appointments, no-shows or appointments canceled with inadequate notice, and returned checks. HSA and FSA cards cannot be used for this purpose. Please note also, that insurance reimbursement is a contract between you and your insurance carrier. I do not accept responsibility for collecting from your carrier on disputed insurance claims, and you are ultimately responsible for full payment on your account.

  • The patient will be charged a fee for service until the deductible is met. For example, if the deductible is $1500.00/year. The patient will pay the out of pocket expenses until the deductible is met.

  • Patients are protected by confidentiality and HIPAA laws.

    Patient information is shared outside the visit if the patient agrees to sign a release of information (ROI).

    Each visit content and all relevant materials to the patient's therapy/treatment will be held confidential unless the patient requests in writing to have all or portions of such content released to a specifically named person/persons. Limitations of such patient held privilege of confidentiality involve client safety, child/elder abuse, or court of law proceedings.

  • Yes. It is important that each patient have an established relationship with a primary care provider for collaboration of treatment when needed.

  • The following is a detailed list of your estimated charges. Since mental health is an ongoing process, this estimate is comprised of the recurring costs of care for 12 months from the date of the estimate issue or until it is replaced by an updated Good Faith Estimate that reflects a better understanding of your treatment needs. This is NOT a contract requiring you to pay the total amount listed, but is an estimate of what services would cost if you utilized them weekly or bi-weekly. You are free to utilize them less frequently than weekly (which would lower your total cost), as well as stop services any time.

    This estimate does NOT include non-medical costs that you as the client have control over and and are therefore impossible to estimate, including court testimony fees, no-show and cancellation fees, records request fees, additional phone calls outside of services. Additional information regarding these fees is found in the Intake Packet, which every new client is required to complete prior to treatment.
